Daily & Monthly COVID Screening
Daily Screening
Please complete Parts I and II of the DAILY SCREENER for each child every morning before they leave for school (parents of students enrolled in all-virtual learning should complete the screening as well). This screening is performed at home and does not need to be submitted to the school. If you answer YES to any of the symptoms listed or any of the screening questions , DO NOT send your child to school. Call the school nurse for instructions on what to do next
Monthly Certification
A parent/guardian must also attest on a monthly basis that the Daily Student Screening above will be performed for each child every morning before school.
